Our Appointment Policy

Thank you for choosing Bay Area Snoring and Sleep Apnea Solutions for your dental sleep medicine, snoring and sleep apnea needs. We are dedicated to providing you with quality care.

We respect our patient’s time and attempt to schedule appointments that are convenient for you. While we always try to stay on schedule, complications and emergencies may arise with other patients that could delay us. If we are significantly delayed, we’ll make every effort to notify you beforehand so you may choose to come later or reschedule. If you are going to be late, please let us know. If you are significantly delayed, your scheduled treatment may be modified or you may be asked to reschedule your appointment.

Because we reserve time exclusively for each patient, we ask that, if possible, you not change your appointment. If you can’t keep your scheduled appointment, we require a minimum of 48 hours notification so we can make your reserved time available for other patients. To notify us of any change, please call our office during business hours or leave us a message.

In order to maintain the most efficient schedule for our patients, our Appointment Policy is as follows:

  • Broken appointments (without 48-hour notification) incur a fee of $100 per scheduled hour.
  • Broken appointment fees must be paid before any further appointments may be reserved.
Your First Appointment

We offer a free sleep apnea consultation to all new patients interested in sleep apnea and snoring solutions. During the consultation we will discuss your sleep apnea and snoring options.

Health History Form

Easily complete our Health History Form from your computer, tablet, or smartphone. If you are unable to complete the form electronically, you may download a hardcopy and bring it with you to your first appointment.

Insurance Information Form

If you’re a new patient using insurance or a returning patient whose insurance information has recently changed, please keep us updated with this form.
